Hypothesis:
High-fidelity simulation-based exercise was recommended by the Accreditation Council for Graduate Medical Education (ACGME) as one method of assessment of core competencies for post-graduate physicians. We developed two simulation scenarios in emergency settings with scenario-specific competency-based assessment to analyze the strengths and weaknesses of novice physician’s performance
Methods:
We conducted a prospective study from March 2018 to January 2020. The simulation curriculum was held once every month with two scenarios exercises. 44 participants’ performances were examined. The core competencies are incorporated into each scenario, focusing on the areas of patient care (PC), medical knowledge (MK), interpersonal and communication skills (ICS), professionalism (PF), and systems-based practice (SBP).
Results:
A total of 156 scenario-specific checklists were collected, of which 46 were from trainers, and 110 were from the trainees. The total scores and the scores of each competence were consistent between the trainees and the faculty. A one-way ANOVA test indicates that there was significant difference between areas, and the scores ranked the highest in PF, followed by PC, MK, SBP, and the lowest is ICS.
Conclusions:
It has been demonstrated that undergraduates need to be formally trained in the concepts of professionalism and ethics, including providing patient condition explanations and showing empathy. Moreover, team communication was not developed during the school period and was not particularly emphasized. Most of the novice physicians can conduct vertical communication (patients and patient's families) well, but horizontal communication with other team members needs to be further developed.
